New Patient Policy

We at Oberlin Road Pediatrics feel very fortunate to be able to provide care to our patients and their families. We are a thriving practice because of the trust you put in us.  We have realized, however, that we have grown to such a point that we are finding it difficult to be able to get our current patients in for well care visits (physical exams) in a timely manner.  As a result, we have come to the difficult decision to stop accepting new patients who want to transfer to our practice.  With this being said, we continue to accept newborns whose families choose us as their primary pediatrician while in the hospital.

Expecting a newborn?

Click newborn for information.  On January 15, 2022 we are ending our practice of seeing babies born at Rex; they will be managed by the Rex hospital newborn team.   We will see all newborns in the 1-3 days after their hospital discharge.

Patient  Forms

Please download, print and complete our office forms prior to your first appointment. If this not possible, then please arrive 15 minutes earlier to complete these in our waiting room.

These additional forms ensure that future visits will go smoothly if your child comes in with someone other than the parent/guardian: Consent By Proxy Decision Maker for Pediatric Care-- This allows your named family member/friend/nanny to bring the child in. Consent by Proxy for Pediatric Care for my Underage Child--  This allows your teenager (ages 16 & 17) to be seen without a parent.
