Please download, print and complete our office forms prior to your first appointment. If this not possible, then please arrive 15 minutes earlier to complete these in our waiting room.
Prior to a newborn's first visit: Newborn first visit packet (7 pages). Please read the Notice of Privacy Practices .
Prior to a child's first visit: New Patient First Visit Packet (8 pages). Please read the Notice of Privacy Practices .
These additional forms ensure that future visits will go smoothly if your child comes in with someone other than the parent/guardian:
Consent By Proxy Decision Maker for Pediatric Care-- This allows your named family member/friend/nanny to bring the child in.
Consent by Proxy for Pediatric Care for my Underage Child-- This allows your teenager (ages 16 & 17) to be seen without a parent.
